Signature Care Management's CareMOSAIC Receives Decision Health Platinum Award

The Bundled Payment team at Signature Medical Group was honored to receive a Decision Health Platinum Award recognizing our custom case management software solution, CareMOSAIC. In addition, we received two honorable mentions for exceptional work in case management in both our orthopedic program and our maternity care program. The Decision Health 8^th Annual Case in Point Platinum Award recognizes individuals and organizations across the country for their success in improving healthcare delivery by empowering patients, improving patient care, and reducing costs.

Our team was honored to receive the award for developing and implementing our innovative IT tool, CareMOSAIC in the category of Health Information Technology Systems for IT Systems that Support Care Transitions. Our software leverages technology to standardize care pathways in episode management and facilitates cost reductions through real-time episode management.

We also received Honorable Mention for Outstanding Achievement in Care Coordination Specialty Programs in both the categories of Care Coordination/Case Management and Women's/Children Case Management. The Signature Bundled Payment team has successfully built and implemented a compelling and successful care redesign model using a multi-faceted strategic approach. Our comprehensive episode management solutions address the needs of patients, providers, and payers to improve outcomes and reduce costs.

Effective case management is achieved through empowering case managers to coordinate care throughout an episode, engaging physicians, utilizing interdisciplinary care teams, addressing behavioral health needs, working with community resources, and utilization of our custom case management software solution, CareMOSAIC.
