For the second year in a row, Signature Care Management was honored to receive a DecisionHealth Platinum Award for our innovative care coordination and data analytics platform, CareMOSAIC. We are also honored that our team member, Nancy Robbins, received an Individual Decision Health Platinum Award recognizing her work as an exceptional case manager. The DecisionHealth Platinum Award recognizes individuals and organizations across the country for their success in improving healthcare through innovation and collaboration. CareMOSAIC facilitates coordination of care across an entire episode incorporating clinical documentation with real-time data analytics and claims data to effectively manage patient care throughout an episode. Our CareMOSAIC platform received the award in the category of Health Information Technology Systems for IT Systems that Support Care Transitions. Nancy Robbins received an individual award in the category of Individual Case Manager/Patient Advocate. Patients are at the heart of the work Signature Care Management does to improve healthcare; having one of our excellent case managers recognized inspires our team to keep improving and driving innovation. In addition to the two awards, our team received two Honorable Mentions for our work in case management for episodes of care. One Honorable Mention was for Outstanding Achievement in Care Coordination/Case Management for our efforts to redesign care and provide effective case management in the Bundled Payment for Care Improvement (BPCI) initiative. A second Honorable Mention was for Outstanding Achievement in Women's/Children Case Management for our successful maternity care program.
